Care Coordination Agencies
The image below is a diagram of how Care Coordination Agencies are working in collaboration with Family Voices Network of Erie County. Family Voices Network is the umbrella over the agencies that are providing care coordination services to families enrolled in the Wraparound Process. The arrows indicate that communication travels between Family Voices Network and the Care Coordination Agencies and from the Care Coordination Agencies back to Family Voices Network. The communications is a two-way process.
The Care Coordination Agencies and Family Voices Network of Erie County share communications regarding families that are enrolled in care coordination. They share data, develop policies and procedures, marketing ideas, parents and youth comments and suggestions, and create plans to address barriers that may exist between the agencies and Family Voices Network. It is a collaborative process. Only a small percentage of families in Wraparound are actually services by these agencies and Family Voices Network tracks all families from intake to enrollment to discharge.

This is a list of the service providers providing services to children, youth and families under the Family Voices Network of Erie County umbrella. Each Care Coordination Agency works with referred families to identify the type of services they need. When a family need services based on their plan of care or needed supports the Care Coordination agencies will find the needed services or supports for the families from the selected service providers who have contracted with Erie County. As of December 2007, this is the listing of our current service providers.
