Who Reviews the Application?
When the referral form is submitted along with the Permission to Disclose Information (parents must be given Informed consent)(signed by the parents), the application is reviewed by the Intake Committee. The Intake Committee includes representatives from:
- Department of Mental Health
- Department of Social Services
- Juvenile Justice
- Family Advocate
This committee meets once a week to review applications to determine eligability. If eligible the family will be assigned to a Care Coordination agency (if there is availability). Family’s are contacted based on the information on the Referral Form. The family’s address & telephone must be accurate; and if necessary please provide us with a backup number to contact the family. This will help remove delays in processing the application and will help us to expediate the application. Normally, families are notified within two weeks of receiving a completed application.
