Here are the qualifications for eligibility
It is important to know who qualifies for this kind of services. The wraparound process is a higher level of care. The wraparound process is for families with children or youth who may be:
- Experiencing Serious Emotional Disturbances (SED),
- Youth who are experiencing emotional, behavioral or social challenges in life, and
- At risk of being removed from their home, school or community because of their mental health needs.
It is also for children and youth who have already been removed from their home, school or community and:
- Placed in a Residential Treatment Facility (RTF), or
- Placed in a Residentail Treatment Center (RTC).
Age requirement:
- The child or youth must be age 5-17 to receive services through the wraparound process.
- However, if they are receiving services before their 18th birthday they can continue to receive services until they reach age 21.
A referral form has to be:
- Completed and mailed to Family Voices Network of Erie County:
- Applications are reviewed on a weekly basis.Consideration, determination and assignment to one of our Care Cooordination agencies.
- When filling out the referral application, the referring agency or person may call for assistance if needed. Simply call (716) 858-2192 for assistance.
- When applications are reviewed a decision is made to determine what level of care a child needs based on the information on the referral form. Referral Forms submitted apply to all services provided by Erie County

It is important that the person referring a family to the Wraparound Process clearly understands how the process work and what is available to the family. Care Coordination is not for all families.
We recognize that sometimes families may be in crisis and contacting the family immediately is important to us. Therefore, it is extremely important that the family’s telephone number and address are accurate on the application at the time of submission; and if necessary, a back-up telephone number is provided.
(Important to include the importance of informed consent (not just a disclosure agreement being signed.)
All forms attached to the Referral Form must be completed, and the family must sign the disclosure agreement that is included in the packet. Without the disclosure agreement we can not process the referral application in a timely manner.
The family income is not a consideration for children experiencing serious emotional, behavioral or social needs in their every day lives. The Wraparound Process supports high-risk children from all walks of life as mental health crosses all boundaries (i.e., social economical, demographic, ethnic, etc.)
